Approved of the agreement between the School Board of Manatee County and Lakewood Ranch Primary Care for clinical experience for students in health occupation fields at Manatee Technical Institute for the 2004-2005 school year. Approved of the agreement between the School Board of Manatee County and Lakewood Ranch Pediatrics for clinical experience for students in health occupation fields at Manatee Technical Institute for the 2004-2005 school year. Approved of the agreement between the School Board of Manatee County and Sunhill Medical Center for clinical experience for students in health occupation fields at Manatee Technical Institute for the 2004-2005 school year. There is no cost to the School District for this clinical experience over and above the insurance policies that are already in effect that cover our nursing students, paramedic students, etc. 20. Approved of Annual Academic Improvement Plan, Language Enriched Plan, and 504 Plan Maintenance Agreement With Synergistic Frameworks, Inc. for the 2004-2005 fiscal year. On June 3, 2002, the School Board approved the purchase and implementation of the Synergistic software system to electronically monitor Academic Improvement Plans, Language Enriched Plans for non-English speakers, and 504 Plans. Included in the purchase agreement was an annual maintenance fee of $20,000.00 to provide technical support and upgrades to the software. Academic Improvement Plans, Language Enriched Plans for non-English speakers and 504 Plans are required by various Federal and Florida rules. The use of these electronic plans are both a time saver for teachers, allowing them to use their time for instructional purposes, and enables the District to ensure compliance for full receipt and retention of Full Time Equivalent funding. The maintenance agreement provides technical support and upgrades to the software for the period of one (1) year. The expenditure for this purchase will be $20,000.00. The requested expenditure for this agreement is included in the Support and Intervention Services budgetary account. 21. Approved of Annual Individual Education Plan Maintenance Agreement With Synergistic Frameworks, Inc. for the 2004-2005 fiscal year. On November 20, 2001, the School Board approved the purchase and implementation of the Synergistic software system to electronically monitor Individual Education Plans (IEP). Included in the purchase agreement was an annual maintenance fee of $25,650.00 to provide technical support and upgrades to the software. IEPs are required by various Federal and Florida rules for students eligible for exceptional student education. These plans must be current to be in compliance. The maintenance agreement provides technical support and upgrades to the software for the period of one (1) year. The expenditure for this purchase will be $25,650.00. The requested expenditure for this agreement is included in the Support and Intervention Services budgetary account. 22. Approved of the Second Amendment to the Lease Agreement between The School Board of Manatee County and The Boys and Girls Clubs of Manatee County, Inc., to extend the Lease Agreement for Horizons Academy through August 1, 2005. In April, 2000 the School Board of Manatee County entered into a contract with the Boys and Girls Clubs of Manatee County, Inc., to occupy and use the facilities located at 1415 9th Street West, Bradenton, Florida, and 5231 34th Street West, Bradenton, Florida. Horizons Academy uses these facilities and adjacent property for classroom portables. On June 18, 2001 the School Board approved an amendment to extend the lease period to June 30, 2004. This Second Amendment extends the Lease for the use of the facilities through August 1 of 2005 and allows Horizons Academy to continue to occupy and use their current site and facilities. The annual expenditure for this lease is $87,500.00. The funding is included in the Support and Intervention Services budgetary account. 23. Approved to contract With Club Z! In-Home Tutoring Services for the provision of Supplemental Education Services for eligible students during the 2004-2005 school year. The federal law, No Child Left Behind, mandated that Supplemental Education Services (SES) be provided to eligible students who attend schools which receive Title 1 funds and have not made adequate yearly progress for two consecutive years. The contract defines the academic objectives identified for the student and the terms and conditions of the service to be provided. Supplemental Education Services will be provided to the student after school hours. The school will receive progress updates during the period of time the student is receiving SES. The service is funded with Title 1 funds specifically set aside for this activity, not to exceed $1,175.00. 24. Approved of the Parent Agreement for Supplemental Education Services through Club Z! In-Home Tutoring Services for an individual student for the 2004-2005 school year. The federal law, No Child Left Behind, mandated that Supplemental Education Services (SES) be provided to eligible students who attend schools which receive Title 1 funds and have not made adequate yearly progress for two consecutive years. The District, as required, provided parents of eligible students with information about state-approved SES providers. A contract was written with the provider selected by the parent. In addition, a parent agreement was written which provided terms and conditions agreed to by the parent, the provider, and the District. The parent agreement form provides the parent with specific information regarding goals, timelines, transportation, attendance, confidentiality and release of information, and other information. It secures parental agreement with the terms and conditions for the SES contract and defines the responsibilities of the parent for the duration of the SES period. There is no fiscal impact. The Title 1 funds for SES are paid directly to the provider by the District. 25. Approved to accept and expend the Comprehensive School Reform Title I, Part F, grant funds (Project 4295) for Tillman Elementary School in the amount of $145,000.00 for the 2004 - 2005 school year. The grant will be used for the implementation of the Literacy First Comprehensive Reading Reform Program and to fund a second reading coach in subsequent years, if additional years are funded by the Department of Education (years 2 and 3).
