There is no cost to the school system for this clinical experience over and above the insurance policies we already have in effect that cover our nursing students, paramedic students, etc. 52. Approved to renew the contract with Children's Haven and Adult Community Services, Inc. for Pre-K disability services for the 2004-2005 school year. A payment of no more than 80 percent of earned Full-Time Equivalency will fund these Exception Student Education instructional services. The estimated cost of this contract is $50,000.00. 53. Approved of the Memorandum of Agreement between Manatee County School Board and Children's Medical Services-Early Intervention Program (CMS-EIP) for the 2004-2005 school year. This Agreement supports the School District's requirement to transition children into the public school system, and will ensure a smooth, seamless transition into school without any interruptions in the child's services. 54. Approved to renew the contract with Ear, Nose and Throat Associates of Manatee, P.A. for the 2004-2005 school year. These services are funded by the Individuals With Disabilities Act (IDEA) grant. The estimated cost of these evaluations is $3,000.00. 55. Approved to renew the contract with Manatee Hearing and Speech for the 2004-2005 school year. These services are funded by the Individuals With Disabilities Act (IDEA) grant. The estimated cost of these evaluations is $5,000.00. 56. Approved to amend the contract for services between Take Care of Manatee, Inc. and the School Board of Manatee County for the 2003-2004 school year. These services are funded by Individuals with Disabilities Education Act (IDEA) grant. The services exceeded the original budget amount by $5,580.00. (Original Contract approved on September 22, 2003.) 57. Approved of the Amendment #2 Agreement HJ0009 (Contractual Services Purchase Order Acceptance Agreement) to renew the agreement between the State of Florida Department of Health and the School Board of Manatee County for the 2004-2005 school year. The Department of Health will reimburse the School Board, on a monthly basis, a total dollar amount specified by the Department of Health not to exceed $30,600.00 in salary for two LPN positions with an additional $10,224.00 to cover salary and benefits paid from Medicaid. (Amendment #2 will change the contract ending date from June 30, 2004 to June 30, 2005, and the phone number for the contact person, Ron Drachler.) (Amendment #1 was approved on August 4, 2003.) 58. Approved to submit the Consolidated Grant Application for Selected Federal Programs for the 2004-2005 school year. If funded, the district will receive grant funds with no matching requirements for a total of $11,823,058.47. 59. Approved to submit the Application for Community High School, Title I, Part D, Subpart 1, Neglected Program grant funds (Project 4415) in the amount of $106,774.55 for the 2004-2005 school year.
