Approval of Purchase(s) Bids Under $100,000.00 - (Continued)

Approval of Renewal of Third Party Administrative Services for the Workers' Compensation and Liability Program

Approved of the contract with Gallagher Bassett Services, Inc. for third party administration of the Workers' Compensation and Liability Program. The estimated expenditure for these services will be $201,176.00 The funds for these expenditures will be charged to the following Budgetary Accounts: Risk Management Fund, Third Party Administrative Services Workers' Compensation and Liability. Approval of Contract with Blue Cross and Blue Shield of Florida, Inc. for Administration of Medical and Prescription Benefits Program and Addition of Optional PPO 1155 Plan, MCSD No. 04-0093-SS

Approved of a three-year contract with Blue Cross and Blue Shield of Florida, Inc. to administer the District's Medical and Prescription Benefits Program, and offer the optional PPO 1155 Plan. The estimated expenditure for these services during Fiscal Year 2004-05 beginning January 1, 2005 will be $1,177,470.00. The funds for these expenditures will be charged to the following Budgetary Account: Risk Management Fund, Professional and Technical Services - $1,177,470.00. Contracts and Grants

1. Approved of Blanche H. Daughtrey Elementary School - Agreement Between Owner and Centex Rooney Construction Company, Inc. - Pre-Construction. The Construction Management Pre-Construction fee for this project is $25,476.00, which is included in the total project budget of $8,986,137.00.

2. Approved of Florine J. Abel Elementary School - Florida Power & Light Permanent Utilities Easement. (Copy of the easement can be found in Supplemental Minute Book.)

3. Approved of H. S. Moody Elementary School - Florida Power & Light Permanent Utilities Easement. (Copy of the easement can be found in Supplemental Minute Book.)

4. Approved of Lakewood Ranch High School - Ninth Grade Center - Amendment 2 - Agreement Between Owner and Willis A. Smith Construction, Inc. - Guaranteed Maximum Price (GMP). Amendment 2 is in the amount of $842,029.00, which brings the total approved GMP to $1,146,029.00 and is included in the total project budget of $1,650,000.00.

5. Approved of contract with Manatee Opportunity Council, Inc., d/b/a Resource Connection for Kids, to collaboratively provide curriculum development and technical assistance to community child care providers for the 2004-2005 school year. The district will receive $117,379.00 from Manatee Opportunity Council to cover salaries, benefits, local area travel, supplies and materials for two school readiness resource specialists.

6. Approved to renew the contract with First United Methodist Church of Bradenton Florida, Inc., d/b/a First for Kids Children's Christian Center, for on-the-job training (OJT) for the 2004-2005 school year. This contract will provide high school Exceptional Student Education students with an OJT program. Cost of transportation for the 2004-2005 school year will be appropriately allocated through the Manatee County School Board Transportation Department budget and Individuals with Disabilities Education Act (IDEA) funds.

7. Approved to renew the contract with Ocean Properties, Ltd., d/b/a Holiday Inn Riverfront, for on-the-job training (OJT) for the 2004-2005 school year. This contract will provide high school Exceptional Student Education students with an OJT program. Cost of transportation for the 2004-2005 school year will be appropriately allocated through the Manatee County School Board Transportation Department budget and Individuals with Disabilities Education Act (IDEA) funds.

8. Approved to renew the contract with Manatee Memorial Hospital, Limited Partnership for on-the-job training (OJT) for the 2004-2005 school year. This contract will provide high school Exceptional Student Education students with an OJT program. Cost of transportation for the 2004-2005 school year will be appropriately allocated through the Manatee County School Board Transportation Department budget and Individuals with Disabilities Education Act (IDEA) funds.

9. Approved to renew the contract with Target Corporation for on-the-job training (OJT) for the 2004-2005 school year. This contract will provide high school Exceptional Student Education students with an OJT program. Cost of transportation for the 2004-2005 school year will be appropriately allocated through the Manatee County School Board Transportation Department budget and Individuals with Disabilities Education Act (IDEA) funds.
