Approval of Purchase(s) Bids Over $100,000.00

Approval to use the Pinellas County School Board Bid for Apple Computer Products, MCSD No. 05-0007-BS

Approved of the use of the Pinellas County School Board bid, to Apple Computer Incorporated, Florida Registration No. 853146, as required, for the period of August 17, 2004 through August 16, 2005, in an amount not to exceed $3,000,000.00. This recommendation includes the authorization to release the $2,000,000.00 budgeted in the 2004-05 budget for the purchase of these computers.

Approval of Request for Proposals for Fresh Delivered Pizza, MCSD No. 04-0108-SS

Approved an agreement for Fresh Delivered Pizza with CiCi's Pizza, Little Caesars - Atris and Kazbour Inc. and Papa Johns Pizza as noted on the Bid Tabulation, for an initial contract period of August 17, 2004 through June 30, 2005. Estimated expenditures will not exceed $150,000.00. Revenues generated from the sale of these items will be approximately $200,000.00. The requested purchases were budgeted for and will be charged to the following account for each cost center: 03-04 School Food Lunch; Food Services; Purchased Food-Direct Shipped; and Food Service Operation. Revenues generated will be credited to the following account for each cost center: 03-04 School Food Lunch; Revenue; Ala' Carte; and Food Service Operation. The three positive respondents are:

Contracts and Grants

1. Approved of the contract between the School Board of Manatee County and Ray Moran for delivery of Adult Career & Technical Education (CTE) Services, under Manatee Technical Institute's Criminal Justice Academy for the 2004-2005 school year. The cost to the school system for Deputy Moran's services will be $27,020.04. He will invoice the district monthly.

2. Approved of the Agreement with Manatee Opportunity Council, Inc., d/b/a Resource Connection for Kids, to provide school-based education services for eligible preschool students for the 2004-2005 school year. The school district will receive a maximum of $582,954.00 to support salaries and benefits for instructional staff and non-direct services such as developmental and nursing screenings, classroom monitoring, and pre/post testing.

3. Approved to renew the contract for services between Take Care of Manatee, Inc. and the School Board of Manatee County for the 2004-2005 school year for the provision of nursing services for medically fragile students at schools on an as needed basis at an hourly rate. The services are funded by the Individuals with Disabilities Education Act grant. The estimated cost of these services will not exceed $50,000.00.

4. Approved to renew the contract with Manatee Glens Corporation for educational services, for the 2004-2005 school year. Funding of salaries for two teaching units are generated through the Florida Educational Finance Program (FEFP). Providing space and services for CSU, Day Treatment and the Elementary Off-Campus (STAR) programs ($210.00 per day x 180 days = $37,800.00), are reimbursed from Individuals with Disabilities Educational Act (IDEA) funds.

5. Approved to renew the contract between Ramsay Youth Services of Florida, Inc., d/b/a Manatee Palms Youth Services, and the School Board of Manatee County for the 2004-2005 school year. The district will receive funding for students based upon Full Time Equivalency projections for the 2004-05 school year. Students receiving services from the agency generate state funds through the Florida Education Finance Program. There are no matching requirements.

6. Approved to renew the contract between Pinnacle Academy, Inc. and the School Board of Manatee County for the 2004-2005 school year. The district will receive funding for students based upon Full Time Equivalency projections for the 2004-05 school year. Students receiving services from the agency generate state funds through the Florida Education Finance Program. There are no matching requirements.

7. Approved to renew the contract between Manatee Glens Corporation, d/b/a Manatee Glens Adolescent Recovery Center (ARC), and the School Board of Manatee County for the 2004-2005 school year. The district will receive funding for students based upon Full Time Equivalency projections for the 2004-05 school year. The district will use these funds to provide the necessary teacher and instructional materials. Students receiving services from the agency generate state funds through the Florida Education Finance Program and Supplemental Academic Instructional categorical funding for juvenile justice programs. There are no matching requirements.

8. Approved to renew the contract between Gulf Coast Marine Institute, Inc. (G.C.M.I.) and the School Board of Manatee County for the 2004-2005 school year. The district will reimburse G.C.M.I. 80 percent of the weighted Full Time Equivalency (FTE) based upon FTE generated during the 2004-05 school year. The maximum number of full-time equivalent students to be funded under this agreement is sixty-five (65). Students receiving services from the agency generate state funds through the Florida Education Finance Program and Supplemental Academic Instructional categorical funding for juvenile justice programs. There are no matching requirements.

9. Approved to renew the contract between the Florida Sheriff's Youth Ranches, Inc., and the School Board of Manatee County for the 2004-2005 school year. The district will receive funding for a minimum of 18 students based upon Full Time Equivalency (FTE) projections for the 2004-05 school year. These funds will be used to provide one Exceptional Student Education (ESE) teacher and one teacher assistant. Additional funding will come from the Title I grant. Students receiving services from the agency generate state funds through the Florida Education Finance Program and Supplemental Academic Instructional categorical funding for juvenile justice programs. There are no matching requirements.

10. Approved of Braden River High School - Utility Easement. This easement will allow Manatee County to access the site, maintain and operate the lift station and related structures at their convenience. (Copy of Easement can be found in the Supplemental Minute Book.)

11. Approved of Manatee Elementary School - Resolution 04-09 - Demolish Duplex Located at 1521 6th Avenue East, Bradenton. The funding to raze the duplex is within the total project budget for Manatee Elementary School. There is no additional fiscal impact to demolish the property. (Copy of Resolution can be found in the Supplemental Minute Book.)

12. Approved of Manatee High School - Ninth Grade Center - Agreement Between Owner and NDC Construction Company - Guaranteed Maximum Price (GMP). The GMP for the project is $254,907.79, which is included in the total project budget of $350,000.00, which includes equipment and miscellaneous construction related costs.

13. Approved of Palmetto High School - Trainable Mentally Handicapped Classroom Addition - Amendment 1 - Agreement Between Owner and NDC Construction Company - Guaranteed Maximum Price (GMP). The GMP for the project, including this amendment, is $78,927.50. The GMP is included in the total project budget of $108,000.00.

14. Approved to renew the contract between Easter Seals Southwest Florida, Inc., and the School Board of Manatee County for the 2004-2005 school year. The district will receive funding based upon Full Time Equivalency projections for the 2004-05 school year. Students receiving services from the agency generate state funds through the Florida Education Finance Program. There are no matching requirements.
