BRHS Sports Orientation
July 31st - 6:30 pm 

For ALL Students wishing to play sports at
Braden River High School next year!

In order to play sports at Braden River all 
athletes MUST have all forms and payments turned 
in to the coach prior to attending tryouts or practice.

The best time and place to do this is at Sports 
Orientation where all forms will be available. 

Please be sure to bring several checks as you will have opportunities 
to purchase Pirate Apparel and Football Sports packages and passes! 
Please list student/athlete's name on any payment. 

The 2012-2013 Forms should be available around July 15th 
for you to download and print them to bring to Orientation.

• FHSAA EL2: Physical Evaluation Form 
• FHSAA EL3: Consent & Release from Liability Form 
• Manatee County School Board Interscholastic 
• Residence Affidavit Form
• Manatee County School Board Mandatory Random 
• Drug-Testing Consent Form
• Mandatory Student Insurance/Athletic Participation

All forms are available at the front office sometime in July/August.
If you are unable to attend the Orientation, you may drop your form out 
at the Front Office along with your insurance payment 
(be sure to list your student's name on all payments).
