to play sports for the 2011-2012 school year.

Drug-Testing Consent Form

The Athlete must have all their forms turned
in to the coach prior to attending practice.

All athletes are required to purchase sports insurance.

The required insurance form should be posted prior to
the Orientation but will also be available at Orientation.
