IMPORTANT CHANGE IN IMMUNIZATION REQUIREMENTS
FOR THE 2012 - 2013 SCHOOL YEAR
Florida Law requires all students in 9th and 10th grade
have a current Tdap shot.
Please make sure your child's shots are up to date.
The Florida State Immunization Form (obtained from your doctor)
must be turned in to the school nurse.
This shot is very important. Thank you.
